The Schleicher County Family Clinic offers a variety of services to take care of the whole family and is open Monday through Friday 8:00 a.m. to 5:00 p.m. with administrative staff also available from 12 noon to 1 p.m. Our medical providers are here to serve your medical needs; however, each provider has a different schedule so for your convenience please call for an appointment. Among the services we offer are annual check-ups, athletic physicals and well-child exams, sick visits, blood pressure maintenance and care, diabetes maintenance and care, blood thinner management, allergy injections, arthritis and orthopedic care, smoking cessation programs and Texas Health Steps exams and child vaccinations.

Management and Treatment of Chronic Medical Conditions

Effective management and treatment of various chronic medical conditions is important to maintaining a healthy, active lifestyle, and the prevention and management of complications.  Some of the chronic medical conditions managed by your healthcare team at Schleicher County Medical Center include:

Treatment and Care of Illness and Injury

Schleicher County Family Clinic is active in the treatment and care of acute illnesses including, but not limited to:

Diagnostic and Screening Tests

Schleicher County Family Clinic offers a wide range of diagnostic tests to assist with the development of your wellness or treatment plan. Services may be provided in the clinic or Schleicher County Medical Center.

Chronic Care Management

The Chronic Care Management Program at Schleicher County Family Clinic was established to help Medicare patients with diabetes, arthritis, high blood pressure, heart disease, stroke and other chronic conditions to play a more active and effective role in the management of their health. If you have a chronic condition, and would like to join the health improvement team, call 325-853-3137 to schedule an Annual Wellness Visit with your primary care provider to discuss enrollment. Participation in the Chronic Care Management program is offered free-of-charge to eligible Medicare patients.

Medicare Wellness Visits

If you are covered by Medicare, you are eligible for a free Annual Medicare Wellness Visit to talk with your provider about ways to improve your health and prevent disease.  The Annual Wellness Visit is not the same as an annual physical exam, but it does provide patients with the same opportunity to talk with their provider about health concerns and to develop a plan in partnership with those providers to focus on health, rather than illness. 

Some of the services covered during an Annual Wellness Visit include:  routine measurements such as weight, height and blood pressure; review of your individual and family medical history, review of your current medications, discussion of the care you receive from other providers, and review of your functional ability and level of safety.  Based on this review, you and your provider will develop a health promotion and disease prevention plan based on your specific needs.

After your initial wellness visit, you are eligible for a follow-up or subsequent wellness visit every 12 months.  These visits are not subject to copayments to deductibles, so there is no cost to the individual. Schleicher County Medical Center and Schleicher County Family Clinic serve all patients regardless of ability to pay. 

Discounts for uninsured patients seeking medically necessary care provided by the hospital and clinic are offered based on the size of the patient’s immediate family, annual household income, and a sliding scale comparing household income with the hospital and clinic’s charity care income guidelines.  

Uninsured patients may apply for a discount in the hospital business office, or clinic front desk.

Our providers contract with most major health insurance companies and participate in the plans these companies offer.  A partial list of insurance companies with which we have contracts can be found below. Please feel free to call us if you have any questions.

If you have medical insurance, please provide that information to our staff when you make your appointment. We will verify your eligibility and file a claim on your behalf.  Providing your insurance information in advance will help speed you through check in at the time of your appointment.

Copays, coinsurance and deductibles are due at the time of service. Not all services, even those most people consider routine, will be covered by every insurance plan. If you have questions about specific benefits, please contact your insurance carrier. Services not covered by your insurance plan are the patient’s responsibility.

Walter "Gordy" Day, M.D.

Gordy Day, M.D., has served on the Medical Staff of Schleicher County Medical Center since 2001. He also operates a private family practice clinic in San Angelo and provides clinical services at the Sterling City Family Clinic in Sterling City, Texas. Dr. Day has worked as a family practice physician in West Texas for over 40 years, as a family practice physician at Shannon Hospital, the emergency room director and staff physician in the Emergency Department of St. John’s Hospital and the Chief of Staff at St. John’s Hospital, all in San Angelo. He earned his Medical Degree and his Bachelor of Science Degree from Southwestern Texas State University, San Marcos, Texas, and completed a family practice internship and residency at the University of Alabama School of Primary Medical Care, Huntsville, Alabama.

Kristina Prater, FNP

Kristina Prater, Nurse Practitioner, joined our medical team at Schleicher County Medical Center in 2021. Kristina was raised in Eldorado and is proud to have the opportunity to serve our community. Kristina earned her Associate of Applied Science in Nursing in 2011 at Angelo State University and began working in rural health as a Registered Nurse shortly after. In 2017 Kristina continued her Education at Angelo State and earned her Bachelor's Degree in Nursing. Kristina continued working in rural health and emergency medicine while pursuing her Master's degree in nursing and became certified as an Advanced Practice Registered Nurse specializing in Family Practice in 2020. Kristina's other medical background includes ICU stepdown nursing, hospice care, hospice administration, and several years working as an Emergency Medical Technician-Basic.

Michael Ross, RN, PA-C

Michael Ross, RN, PA-C, has worked as a physicians assistant since graduating from the Primary Care Program, Stanford University School of Medicine, Stanford, California, in 2004. Prior to joining Schleicher County Medical Center and Schleicher Family Clinic, he worked in the emergency departments and inpatient units of hospitals in Eden, Quanah, Brady and Abilene, Texas. He also worked in primary care and specialty clinics in Tyler, Texas.

Kimberly Babb, FNP

Kimberly Babb, Nurse Practitioner, joined our medical team at Schleicher County Medical Center in 2022. Kimberly was raised in Mertzon and is proud to have the opportunity to serve our community. Kimberly has been working in healthcare for over 20 years. She first started out as a CNA then received her ER tech certification. Kimberly worked at Shannon Medical Center for several years while going to school to become an LVN. Kimberly received her RN Associates degree in 2008 from Angelo State University and began her journey into Labor and Delivery. In 2013 she received a bachelor’s in nursing degree from the University of Texas at Arlington and graduated in 2016 from Walden University with a Master of Science in Nursing degree as a Family Nurse Practitioner. Upon graduation Kimberly took a Hospitalist job in Vernon, Tx and then went on to Quanah, Tx to work in their hospitalist program and Emergency room. Kimberly returned to this part of Texas to be near family and is excited to care for our community as she would her own family.
